By agreeing to participate in a Team Tui trial, you are agreeing to the following terms and conditions:
I will read the information provided on the trial and consider whether or not to participate in the trial.
I understand that I am free to ask for any further information at any stage.
If I apply, I confirm that I will be a willing participant in the service trial and the evaluation and I understand my responsibilities as a trial participant.
I understand that taking part in a trial is voluntary (my choice) and that I may withdraw from a trial at any time without this affecting any services that I might be receiving from St John.
I acknowledge that I may be required to provide personal information (e.g. mobile phone number) to St John to produce a personal profile and I agree to provide the relevant information when requested.
I consent to the evaluation team collecting and processing my information.
I understand that I may be interviewed (e.g. survey, phone call or friendly chat in person) by the evaluation staff as part of the trial.
If I decide to withdraw from the trial, I agree that the information collected about me up to the point when I withdraw may continue to be processed.
I understand that no material which could identify me personally will be used in any reports on the trial.
